Parent and Guardian Consent for Counseling and Health-Related Services
In compliance with Senate Bill 12 (SB 12), Valley View ISD must obtain written consent from a parent or guardian before school nurses, athletic trainers, or counselors can provide certain services to students. These services include routine health care and counseling support that were previously allowed without written permission.
If you wish for your child to continue receiving all counseling and health-related services, please complete the consent forms below.
A separate form must be submitted for each student.
Please note that a delayed submission may result in interruptions or delays in services for your child.

Background on Senate Bill 12
In 2025, Governor Greg Abbott signed Senate Bill 12, which requires school districts across Texas to obtain written parental consent before providing routine counseling and health-related services.
This law takes effect on September 1, 2025.
Examples of Counseling Services Requiring Consent

Brief support or strategies for issues such as stress, anxiety, peer relationships, or short-term emotional challenges

Goal setting and decision-making guidance

Check-ins with students for self- or parent-initiated concerns

Examples of Health-Related Services Requiring Consent

First aid and injury evaluation

Monitoring chronic conditions such as asthma or diabetes

Support during illness or injury while at school

Administration of medications (with parental request and physician order)
Without signed consent, staff may only provide limited services, such as in medical emergencies or situations involving immediate danger, including suicidal thoughts.
As required by law, Valley View ISD staff will continue to respond to and report suspected abuse or neglect regardless of consent status.
