• How to Request a 1095-B Form: – 

  Recent changes under the Paperwork Burden Reduction Act have modified how employers distribute 1095-B forms. Employers are no longer required to mail these forms to individuals as proof of minimum essential coverage unless specifically requested.

  If you need a paper copy of your 1095-B form, please email Misty Price at mprice@vvisd.net.
