UIL Pre-Participation Physical Examination Form:
It is recommended that all athletes have a physical examination every year of athletic competition. However, as a minimum requirement the UIL only requires physical examinations when students are entering the 7th, 9th, and the 11th grades. Transfer athletes or athletes new to athletics would be required to have a physical examination their first year in athletics at Valley View ISD and then follow the minimal schedule from that point. A UIL Pre-Participation Physical Examination Form must be completed and signed by either a Physician, a Physician Assistant licensed by a State Board of Physician Assistant Examiners, a Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse Examiners, or a Doctor of Chiropractic. Examination forms signed by any other health care practitioner, will not be accepted. This form is located on the Athletics website or can be picked up in the athletics office.
UIL Medical History Form:
Each year prior to any practice or participation a UIL Medical History Form signed by both a student and a parent or guardian is required. A Medical History Form shall accompany each physical examination and shall be signed by both a student and a parent or guardian. This form is the first page of the Pre-Participation Physical Examination Form.
The Valley View Family Medical Clinic will be conducting physicals for Valley View ISD athletes on the following schedule.
June 17th 12:00 Junior High Boys
June 18th 12:00 High School Boys
June 23rd 12:00 High School Girls
June 24th 12:00 Junior High Girls
The physicals will be conducted at the clinic for a cost of $20. Appointments are not needed.
